BRAVE (Breaking the Cycle of Violence with Empathy) Case Manager

We currently have a Temporary Full-Time BRAVE (Breaking the Cycle of Violence with Empathy) Case Manager opportunity until approximately September 2021 in the Tory Trauma Program at our Bayview campus.

The BRAVE Case Manager is charged with implementation of the BRAVE Project, a hospital-based violence intervention program, which serves high-risk patients with violent injury. With an in-depth knowledge in addressing root causes and risk factors of violence, the BRAVE Case Manager aims to focus their efforts on the following outcomes:

  • Facilitating referral to youth violence and youth justice programs and other community services including, but not limited to: Toronto FOCUS (Furthering Our Community by Uniting Services) to address acutely elevated risk status of patients in a community context and to ensure a comprehensive response to risk factors contributing to violence such as education, employment, and housing.
  • Linking patients and families with mental health support (e.g. counselling for PTSD, anger management, general mental health, substance misuse, spiritual care).
  • Ensuring patients are linked with medical provider(s) to provide follow-up treatment of violent injury and ongoing health care.

The BRAVE Case Manager is an early interventionist at Sunnybrook Health Sciences Centre’s Tory Trauma Program when youth and young adults have been injured by violence in the City and area surrounding Toronto. They provide case management and provide links for individuals to important city-wide resources in order to reduce the risk of re-injury. When patients are identified as potential participants, the Case Manager will screen for enrollment, and play the role of mentor to study participants, connecting them to services and resources after discharge and continue to provide case management for up to 6-12 months.

The BRAVE Case Manager will collect and analyze data that is later used to establish best practices in violence prevention, as well as to assist in the development and sustainability of community resources. The BRAVE Case Manager aims to stop the revolving door of injury by addressing the root causes of violence at both the community level and in influencing public policy. Reporting to the Manager, Trauma Services, the BRAVE Case Manager is an integral part of the Centre for Injury Prevention and the Tory Trauma Program. The incumbent is expected to exercise strong familiarity and knowledge of case management principles and practices and have awareness of community resources and services available for at-risk youth.
