HERE’S HOW THE CREATORS DESCRIBE THIS TOOLKIT:

The journey from foster care to independence can been difficult, confusing, and uncertain times. Like any other journey, being prepared helps to set the right course and avoid getting lost on the way. Today more than ever, if you want success (and safety) to live on your own, you’ll need to plan for your trip.

FosterClub’s Transition Toolkit was designed just for this transitioning journey. It is a youth-involved, comprehensive way to develop a transition plan for foster youth aging out of care. The Transition Toolkit includes planning worksheets, record keeping, detailed maps, and resources on the topics of:
